ABSTRACT Background: The long-term effects of schistosomiasis on the glomerulus may contribute to the development of chronic kidney disease. This study aimed to investigate baseline Schistosoma mansoni-Circulating Anodic Antigen (CAA) levels and their association with kidney biomarkers related to podocyte injury and inflammation in long-term follow-up after praziquantel (PZQ) treatment. Methods: Schistosoma infection was diagnosed by detecting CAA in urine using a quantitative assay based on lateral flow using luminescent up-converting phosphor reporter particles. A cutoff threshold of 0.1 pg/mL CAA was used to diagnose Schistosoma infection (baseline) in a low-prevalence area in Ceará, Northeast, Brazil. Two groups were included: CAA-positive and CAA-negative individuals, both of which received a single dose of PZQ at baseline. Urinary samples from 55 individuals were evaluated before (baseline) and at 1, 2, and 3 years after PZQ treatment. At all time points, kidney biomarkers were quantified in urine and adjusted for urinary creatinine levels. Results: CAA-positive patients had increased baseline albuminuria and proteinuria and showed greater associations between kidney biomarkers. CAA levels correlated only with Vascular Endothelial Growth Factor (VEGF) (podocyte injury) levels. Increasing trends were observed for malondialdehyde (oxidative stress), monocyte chemoattractant protein-1 (inflammation marker), and VEGF. In the follow-up analysis, no relevant differences were observed in kidney biomarkers between the groups and different periods. Conclusions: S. mansoni-infected individuals presented subclinical signs of glomerular damage that may reflect podocyte injury. However, no causal effect on long-term renal function was observed after PZQ treatment.

ABSTRACT Bibliometrics is a branch of scientometrics that allows the study of scientific activity. Its unit of analysis is the scientific article. The interest in this area of research lies in its implication in decision making, as part of knowledge management, by suggesting the development of lines of research that have been little studied or supervising the development of others and evaluating the scientific work of authors, institutions and even countries. The central theme of this article is to analyze bibliometrically the scientific production related to academic achievement in the university context. The approach used was quantitative, because it used statistics to give the desired result. The type of research was descriptive because it sought to characterize bibliometric indicators such as year of publication, country where the study was carried out, journal of publication, main topics, methodologies and areas of knowledge used. A total of 145 scientific articles related to the scientific production on human talent and academic achievement published in the Scopus databases were taken into account, corresponding to the period sampled between 2006-2021.

BACKGROUND: In South Africa, initiating and managing insulin in primary care for people living with type 2 diabetes (PLWD) is a major challenge. To address these challenges, a multidisciplinary team from the University of Pretoria (South Africa) developed the Tshwane Insulin project (TIP) intervention AIM: To determine internal and external factors, either facilitators or barriers, that could influence the implementation of the TIP intervention and propose strategies to ensure sustainability SETTING: Tshwane District, Gauteng province, South Africa METHODS: We used the SWOT framework to qualitatively analyse the strengths, weaknesses, opportunities, and threats influencing the implementation of the TIP intervention. Four field researchers and three managers from the TIP team participated in an online group discussion. We also conducted semi-structured interviews with healthcare providers (HCPs) (seven nurses, five doctors) and patients with type 2 diabetes (n = 13 RESULTS: Regardless of the identified weaknesses, the TIP intervention was accepted by PLWD and HCPs. Participants identified strengths including app-enabled insulin initiation and titration, pro-active patient follow-up, patient empowerment and provision of glucose monitoring devices. Participants viewed insulin resistance and the attitudes of HCPs as potential threats. Participants suggested that weaknesses and threats could be mitigated by translating education material into local languages and using the lived experiences of insulin-treated patients to address insulin resistance. The procurement of glucose monitoring devices by national authorities would promote the sustainability of the intervention CONCLUSION: Our findings may help decision-makers and health researchers to improve insulin management for PLWD in resource-constrained settings by using telehealth interventions

ABSTRACT Objective. To describe the needs of academic staff conducting non-communicable disease (NCD) research at the University of the West Indies, Mona Campus in Jamaica. Methods. Utilizing a cross-sectional design an online survey was created using the research electronic data capture application (REDCap); it was disseminated via email to 708 academic staff members in the Faculties of Medical Sciences and Science & Technology between September and November 2018. Participants were asked to indicate their level of access to expertise, training and equipment for conducting research. Descriptive analysis was conducted using STATA version 14. Results. Most respondents were women (74.2%), predominantly scientists (33.1%) or specialist physicians (22.6%). Less than 2/3 of respondents reported publishing research findings in peer reviewed journals, with a quarter not disseminating their research findings in any medium. Resources for field research/data collection, epidemiological methods and principles, and data management/data analysis were generally available. However, there was limited access to training, expertise and equipment in emerging techniques for NCD research such as metabolomics, bioinformatics/analysis of large-scale data sets and health economics. Additional challenges included limited access to financing for research, inadequate workspace and poor administrative support for conducting research. Conclusions. There is a need for more local research seed funding, stronger administrative support for researchers, and opportunities for training in cutting edge NCD research techniques. Jamaican researchers could benefit from being part of a regional research centre of excellence with critical research skills and equipment that builds research networks and strengthens the NCD research response.

BACKGROUND: In South Africa, the Central Chronic Medicine Dispensing and Distribution (CCMDD) programme allows stable patients with non-communicable diseases, including type 2 diabetes mellitus (T2DM), to collect their medication from a pick-up location near their home, thus avoiding long waiting times and travel expenses. The CCMDD programme aims at improving patient retention and adherence through better access to medicines, resulting in better health outcomes. AIM: We assessed whether patients with T2DM enrolled in CCMDD achieved the recommended targets for glycaemic, blood pressure (BP) and lipid control. SETTING: City of Tshwane, South Africa. METHODS: We reviewed the records of 198 T2DM patients enrolled in CCMDD and assessed their control of haemoglobin A1c (HbA1c), BP and lipids. RESULTS: Most of the records reviewed belonged to women (64.7%), African (89.9%), hypertensive (82.7%) and to patients exclusively on oral antidiabetic agents (98.5%). Patients were, on average, 57.7 (s.d. = 12.1) years old and had participated in the CCMDD programme for, on average, 2 years. The mean HbA1c was 8% (s.d. = 2). Glycaemic control was achieved by only 29.2% of patients, and 49% of patients had HbA1c between 7% and 9%. Ninety-three patients (66%) had achieved the total cholesterol target, 57.4% achieved BP targets and 6.9% had achieved the low-density lipoprotein cholesterol target. CONCLUSION: A small group of patients achieved the targets for glycaemic, BP and lipid control. Despite improved accessibility to medication, the CCMDD is not synonymous of improved clinical outcomes. Future research should ascertain the factors associated with suboptimal control for these patients.

Abstract: Objective: Evaluate the effect of spatial repellency against Ae. aegypti of two chemical compounds impregnated in different types of fabrics. Materials and methods: The study was carried out in the year 2015-2016 in the Centro Regional de Investigación en Salud Pública, del Instituto Nacional de Salud Pública. The high-throughput screening system was used to evaluate the response of Ae. aegypti to transfluthrin and linalool, impregnated individually at different concentrations in poplin, cotton and polyester. The effect of their mixtures was also determined, washing on residuality and percentage of protection. Results: The highest spatial repellency response was for 0.1% linalool-cotton treatment (RE = 70 ± 5.77%). The mixture of 0.1% linalool and 0.001% transfluthrin presented a similar spatial repellence percentage for the three types of fabric. The transfluthrin-poplin treatment 0.001% maintained a residual of five days. 0.1% linalool produced a 62.50% protection in the presence of an attraction stimulus. Conclusion: It is suggested the impregnation of 0.1% linalool in clothing as a protection measure for Ae. aegypti.

Abstract This research aims to contribute to the knowledge about the minimization of cyanide consumption in gold mining, mainly in complex minerals such as gold sulphides and gold auroarseniides; with the use of oxidizing salts. Cyanidation assays were performed varying the cyanide concentration, aeration, pH and particle size; salts of lead acetate and lead nitrate were used as passivants. In conventional cyanidation the reagent consumptions for these minerals are high; the use of passivating salts results in a reduction in the consumption of cyanide and other reagents, under the same control parameters and mineral overmolding process. Overmolding (closed milling circuit) is the process in which the mill uses a classifier whose thick product returns to the mill, and whose fine product passes to the next separation stage. This is applied after the metallurgical characterization of the sample by microscopy, observing the partial occlusion of the gold material in pyrrhotite matrix whose treatment requires high amounts of alkali and cyanide. The treatment with lead salts (passivation salts) that superficially alter the structure of the gold sulphide mineral was applied, with a decrease in the interaction with alkali and cyanide and consequent decrease in its consumption. As a result of the cyanidation process with salts, a gold solution of 97.78% was obtained for 24 hours, in significant contrast to the conventional treatment without passivation salts that gave a gold solution of 84.50%. In another cyanidation experiment, a gold solution of 93% was obtained within 24 hours, from the ore pre-oxidized with air, while the auric solution was only 80% in the same period from ore without pre-oxidation. In conclusion, a higher recovery yield of precious metal came from a pre-treatment of the mineral with passivation salts or by the aeration method, resulting in a good degree of particle release, with the consequent decrease in the consumption of cyanide and alkali.

Abstract Introduction: For a few years, education has witnessed changes in training. We try to go from a training based on the passive acquisition of theoretical knowledge to another more active, dynamic and reflective formation. The ideal way to assess professional ability is to observe what the doctor does in the usual situations in his or her practice. As this involves great technical difficulties, instruments that closely approximate reality have been designed, such as Structured Objective Clinical Evaluation. Objective: The objective was to apply a structured objective clinical evaluation to assess the clinical skills of fifth-semester students. Method: A test committee was formed by four teachers from the general medical training area. A table of specifications of clinical skills was made to evaluate students with planned clinical cases, including anamnesis, physical examination, clinical judgment, anatophysiological diagnosis, imaging skills and therapeutic communication. Four stations were designed, their content was validated through expert judgment and the predictive validity of the checklists was calculated. The test was applied to 17 students, the instrument was validated, results of the descriptive statistics were obtained and the Cronbach's alpha was calculated. Results: The analysis of agreement of the evaluation instrument according to the experts was acceptable because the condition of essential exceeded 80%; Predictive validity and Cronbach's alpha in three of the stations was significant, chagasic heart disease was the best evaluated with 97.5 and the general Cronbach's alpha was 0.507 Conclusions: The instruments used in structured objective clinical evaluation have sufficient validity and reliability to be applied in the research context and improve the quality of clinical evaluations, making them more objective and with greater possibilities of feedback in the learning process.

The changing of place names (e.g. street and public buildings) often accompanies change of governments, particularly in countries where one ideology has triumphed over another. This was the case in Zimbabwe following the triumph of ZANU-PF over the settler colonial government of Ian Smith, in 1980. Harari Township, the oldest black suburb in the then city of Salisbury (also renamed Harare), underwent name change in 1982 and was renamed Mbare. Public consultations by the City Council resulted in the changing of the street names that brought about a changeover in Township's ondonyms. Streets and public buildings were renamed after persons who had made outstanding contribution to the development of the Township. Among those honoured were two outstanding female Christian leaders, Mrs Elizabeth Maria Ayema (popularly known as Mai Musodzi) and Barbra Tredgold. Incidentally, these were the only two women after whom a street and a public amenity were named. In this article, we investigate the contribution that these two women made to Mbare Township to deserve the honour bequeathed on them by the residents of Mbare. In the article, we acknowledge that Mai Musodzi and Barbara Tredgold were honoured because they were among the illustrious leaders, who served the Township with distinction. By honouring their memory, residents of Mbare were, by implication, making a commitment to live by the values that the two stalwarts stood for. In our conclusion, we argue that the tribute accorded to the two reflects the consensus of the residents of Mbare that Christian values that the two had lived by were an important site of struggle for marginalised black people, who made the ghetto-like 'Location' a homely habitat.

BACKGROUND: In South Africa, much of diabetes care takes place at primary healthcare (PHC) facilities where screening for diabetic complications is often low. Clinics require access to equipment, resources and a functional health system to do effective screening, but what is unknown is whether these components are in placeAIM: The aim of this study was to assess the capacity of primary care clinics in one district to provide quality diabetes careSETTING: This study was conducted at the Tshwane district in South AfricaMETHODS: An audit was done in 12 PHC clinics. A self-developed audit tool based on national and clinical guidelines was developed and completed using observation and interviewing the clinic manager and pharmacist or pharmacy assistantRESULTS: Scales, height rods, glucometers and blood pressure machines were available. Monofilaments were unknown and calibration of equipment was rare. The Essential Drug List was the only guideline consistently available. All sites reported consistent access to medication, glucose strips and urine dipsticks. All sites made use of the chronic disease register, and only 25% used an appointment system. No diabetes-specific structured care form was in use. All facilities had registered and enrolled nurses and access to doctors. Availability of educational material was generally poorCONCLUSION: The capacity to deliver quality care is compromised by the poor availability of guidelines, educational material and the absence of monofilaments. These are modifiable risk factors that could be resolved by the clinic managers and staff development educators. However, patient records and health information systems need attention at policy level

Introduction: During the last decades there has been a gradual increase of cardiovascular diseases, who have become the main cause of death in the world and especially in Latin American countries, despite having identified preventable cardiovascular risk factors that explain around 90% of myocardial infarctions and cerebrovascular accidents. Therefore, it is require to implement actions that will allow to control cardiovascular risk factors, including being overweight or obese. Motivation: To review the effect of GLP-1 receptor agonists, including liraglutide, in controlling cardiovascular risk factors, particularly excess weight and obesity and the subsequent prevention of cardiovascular diseases. Methods: Evidence were searched by means of boolean operators and cross references in databases MEDLINE, LILACS and Google Scholar. Conclusions: GLP-1 receptor agonists can be a pharmacological alternative to control excess weight and obesity, and may help in preventing cardiovascular disease, though some questions remain regarding the benefits of its use in our field.

Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.
